EASY TASK FOR A WEB FRONTEND PROFESSIONAL (BUILD 1 PAGE WITH 3 SECTIONS)

1- This page is for an small clinic web application (PHP) that I built; but I'm considering a frontend re-design.
2- Please check the image attached, I need something similar.
2- As you can see in the image, the page has 3 sections, one section on the top (full width horizontally), which has the patient information, and 2 sections at the bottom; the left section (patient clinical history) has a form and the right section has some tabs (medical records); my intention is to place some tables and forms on those tabs.
3- The page must be responsive.
4- I built the old application, I used the html and bootstrap template ADMINLTE v.
